Welcome to Pace University! We hope your summer is going well and you are preparing for the upcoming academic/athletic year. This letter is to pass along some information and paperwork for you and your parents to complete. Please take care in providing all the information asked for.

1. INSURANCE PACKET AND INFORMATION
Please read over this information with your parents/legal guardians. Fill out the form and return it with copies of the card enclosed. All sections must be completed and signed. DOWNLOAD FORM (PDF)

**NOTE: Parent’s signature is REQUIRED to complete this form.

2. SPORTS MEDICINE HISTORY QUESTIONNAIRE
This packet will take a few minutes to complete, but please be sure to answer all the questions and sections. In this packet is the questionnaire, medical authorization, release of information, shared responsibility and physical examination form. DOWNLOAD FORM (PDF)

3. ATHLETIC PHYSICAL FORM
The physical examination must be completed by your family physician. This is the physical for your athletic participation. You will be given an additional orthopedic exam upon arrival. You will be notified of the exact time and location of the screening when you arrive on campus for the fall semester. DOWNLOAD FORM (PDF)

All paperwork must completed in full and returned to the Sports Medicine Department before the first day of practice. Any paperwork that is incomplete or not returned by this date will delay your processing and may delay your privilege to participate.
